The steering wheel knob "lazy" is a small, independently rotating lever facing the driver and is securely mounted on the outer rim of the steering wheel.
The protruding lazy is an aftermarket accessory. The free rotation is intended to facilitate driving by quickly turning the steering wheel with one hand.
Steering wheel knobs were quite popular in trucks and tractors before the advent of power steering.
Their main use today is still in trucks, especially in vans, where they allow simultaneous driving & shifting.
They are also used in forklift lifting lawn mowers where frequent sharp turns are required, but also by many car drivers.
